LPN/LVN for RPM (Bilingual, Spanish Speaking)

RPM Healthcare is a fast-growing company with nearly 40 years of patient education experience. Our proprietary software tracks patients’ physiological data (Blood Pressure, Weight, Blood Glucose, Lung Capacity, Oxygenation), identifying dangerous out-of-range values and trends that impact health. As a bilingual (Spanish) speaking RPM 365 Care Coach, you’ll engage patients, track and analyze their data, provide education, and encourage adherence to their care plans. About You You're a licensed LPN/LVN passionate about healthcare innovation and patient outcomes. Your tech-savvy, comfortable with high-volume phone outreach (125+ conversations a week), and thrive in a fast-changing environment. You’re patient, empathetic, detail-oriented, and a self-starter.
Responsibilities
- Serve as the primary contact for patients on the RPM365 platform.
- Assist with device setup and app connectivity.
- Engage with patients to ensure adherence to care plans.
- Schedule monthly check-ins and monitor patient data.
- Educate patients through shared resources.
- Notify healthcare providers of critical alerts.
- Attend virtual team meetings for updates.
Qualifications
Minimum:
- Active nursing license.
- Bilingual, Fluent in Spanish
- Knowledge of chronic conditions and related monitoring devices.
- Knowledge of remote patient monitoring CPT code qualifications.
- Proficiency with technology, computer platforms, and smartphones.
- Strong verbal/written communication skills (English and Spanish)
- Comfortable with patient outreach and engagement (English and Spanish)
- Highly organized, detail-oriented, and team-focused.
- Independently motivated
- Comfortable spending the majority of your workday engaging with patients over the phone providing support, education and guidance.
Preferred:
- Experience in telehealth, remote patient monitoring, education, and/or customer service.
- Experience in weight management coaching
- Additional language skills are a plus.
Compensation & Benefits
- Salary: $45,000-$49,000 (DOE) + performance bonuses.
- Schedule: Mon-Fri, 9:00 AM - 5:00 PM, EST (Remote).
