What To Expect (Job Responsibilities)
- Conduct targeted patient outreach to promote completion of recommended care and preventive screenings
- Schedule appointments and coordinate follow-up care to ensure timely patient services
- Identify and close care gaps by collaborating with clinic teams and referral partners
- Prepare tools and documentation for providers to enhance performance during patient visits
- Maintain organized work queues and participate in team meetings for continuous improvement
What Is Required (Qualifications)
- LVN, Certified Medical Assistant, or equivalent clinical experience required
- 2-4 years of experience in a clinical or physician office setting
- Proficiency with EMR systems, preferably Athena
- Strong organizational and communication skills
- Ability to manage multiple priorities in a fast-paced environment
How To Stand Out (Preferred Qualifications)
- Experience in value-based care, quality improvement, HEDIS, or care coordination workflows
- Familiarity with patient outreach and population health initiatives
- Knowledge of Medicare and Commercial value-based programs
