REMOTE Care Coordinator (bilingual Korean)

Care Coordinator (Remote – New York State Residents Only) Make a Meaningful Impact Through Personalized Care Navigation We are seeking compassionate, resourceful, and motivated Care Coordinators to support members throughout New York, particularly those facing social, behavioral, and environmental barriers that affect their health and overall well-being. In this remote role, you will use Motivational Interviewing (MI) techniques to build trust, empower individuals, and connect them with essential community resources that help improve their quality of life. This is an excellent opportunity for professionals passionate about health equity, social determinants of health, and helping underserved populations navigate complex systems of care. Candidates must reside in New York State. Occasional travel within New York may be required for program-specific training and team meetings.

Key Responsibilities

  • Utilize Motivational Interviewing techniques to engage members, establish trust, and support behavior change.
  • Conduct outreach and ongoing engagement through phone, text, and video communication.
  • Assess member needs related to housing, food security, transportation, behavioral health, healthcare access, and other social determinants of health.
  • Connect members with appropriate community resources, public assistance programs, and supportive services.
  • Coordinate care and collaborate with interdisciplinary teams, including Dietitians, Clinical Partners, and Member Support staff, to ensure seamless care transitions and warm handoffs.
  • Develop, document, and maintain accurate care plans and member records using internal systems and digital platforms.
  • Monitor member progress, identify barriers to success, and provide ongoing encouragement and problem-solving support.
  • Deliver services using trauma-informed, culturally responsive, and person-centered approaches.
  • Maintain compliance with organizational policies, confidentiality standards, and documentation requirements.

Qualifications

  • Minimum of 2 years of experience in Care Coordination, Care Navigation, Social Work, Health Coaching, Behavioral Health, Community Health Work, Case Management, or a related field.
  • Must reside in New York State.
  • Fluency in Spanish, Korean, or Russian is required.
  • Experience utilizing Motivational Interviewing or similar evidence-based engagement techniques.
  • Knowledge of community resources and public assistance programs, including Medicaid, SNAP, housing assistance, behavioral health services, and related support systems.
  • Experience working with low-income populations, individuals with chronic health conditions, or underserved communities.
  • Strong communication, relationship-building, and problem-solving skills.
  • Comfortable working independently in a remote environment while managing multiple member cases.
  • Proficiency with CRM systems, EHR platforms, and other digital documentation tools.
  • Community Health Worker (CHW) certification is a plus.

Preferred Qualifications

  • Experience navigating New York City and New York State social service systems.
  • Familiarity with social determinants of health and health equity initiatives.
  • Experience supporting culturally diverse and multilingual populations.
