Associate Director, Care Management
Become a part of our caring community Oversee the assessment and evaluation of member's physical and behavioral health needs to achieve and maintain wellness by guiding members and their families/caregivers toward appropriate for the member's care and wellbeing. Guide the development, implementation, and evaluation of care management programs to align organizational goals and regulatory requirements. Lead multiple managers and highly specialized processional associates Monitor data sharing among internal and external resources to ensure service integration, continuity of care, and contractual compliance, addressing challenges, risk, and opportunities. Facilitate communication and collaboration with internal and external stakeholders, including providers, payers, vendors, and regulatory agencies. Lead the preparation and presentation of program reports, using data analytics to monitor performance, identify trends, and recommend improvements. Champion initiatives for process improvement and innovation to advance care management practices and outcomes. Use your skills to make an impact Required Qualifications: Bachelor's Degree Active Registered Nurse (RN) license or Social Work (SW) license Must be willing to travel into IL as needed for meetings, audits, or other departmental needs. 6 or more years of professional experience 2 or more years of previous formal leadership/management experience Proficiency in analyzing and interpreting data trends Progressive business consulting and/or operational leadership experience Preferred Qualifications: Advanced degree in nursing, health science, or business-related field Previous experience working in a managed care field 5 or more years of previous management/supervisor level experience As part of our hiring process for this opportunity, we will be using an interviewing technology called HireVue to enhance our hiring and decision-making ability. Work from a dedicated space lacking ongoing interruptions to protect member PHI / HIPAA information. Travel: While this is a remote position, occasional travel to Humana's offices for training or meetings may be required. Scheduled Weekly Hours 40 Pay Range The compensation range below reflects a good faith estimate of starting base pay for full time (40 hours per week) employment at the time of posting. The pay range may be higher or lower based on geographic location and individual pay will vary based on demonstrated job related skills, knowledge, experience, education, certifications, etc. $104,000 - $143,000 per year This job is eligible for a bonus incentive plan. This incentive opportunity is based upon company and/or individual performance.
